- Revealing mistakes: When the Blazer flips over, you can see the mechanism under the truck on the passenger side used to flip the truck over.
- Continuity: Joan’s hand moves, disappears and reappears from Frank’s face between shots during the “memories” speech,
- Continuity: When Castle grabs the motorcycle to chase after the gunmen who are chasing Maria and Will, the dead family member who’d originally been riding it is nowhere to be seen.
- Continuity: As the gunmen are driving off the pier after leaving Castle for dead, the film is flopped.
- Continuity: When the guitar player comes into the diner, he sits down and, while smoking his cigarette, opens his guitar case. As he does so, his cigarette repeatedly jumps back and forth from his mouth to his hand between shots.
- Continuity: When Quentin shows Castle’s classified dossier to Howard Saint it shows Castle’s date of birth as 1972 but later when we see the gravestone it shows his date of birth as 1968.
- Revealing mistakes: In some of the shots after Frank is sewn up by Joan, you can see the edges of the prosthetic stab wound on his chest.
- Factual errors: The truck that is used to kill Castle’s family (in Puerto Rico) has a front license plate. License plates in Puerto Rico are only displayed in the back of the car, as you are only given one plate for the car.
- Continuity: When Frank’s father is giving his speech to the family, he spills his drink on Frank’s lap which causes Frank to look down. In the next shot, Frank is looking up at his father.
- Continuity: Frank’s shirt is dry immediately after his father spills drink on it.
- Continuity: The second time Frank places a fire hydrant by Mrs. Saint’s car, he is briefly wearing the short sleeved Skull t-shirt (from the previous time he did it), but in the wider shot he is wearing the long sleeved trench coat.
- Continuity: When Frank is being held by Saint’s men on the pier, one shot shows his dead wife lying on her back, the next shot shows her lying on her side.
- Continuity: When Frank is being chased by the Heck (after he gets over the bridge), it shows the metal shields are pulled down on both doors, but in a shot when it shows him turning, you can see one of the shields is no longer there, but in the next shot, it is back.
- Continuity: During the shootout at the family reunion, one of Saint’s men is shooting from behind a barbecue grill at Frank and Frank’s father. As Frank and his father are shooting at Saint’s man, a bowl of salad on the counter is sent flying in the air as Saint’s man dives over the barbecue grill. The next shot shows Saint’s man shooting at Frank with the salad bowl full of salad on the counter. Then we see Frank shooting at the barbecue grill with the salad bowl gone and salad all over the counter.
- Revealing mistakes: As Castle and Lincoln fight, and Frank lifts the knife to Lincoln’s face, right before Frank cuts him, you can see the tape across the face of where Lincoln is to be cut.
- Continuity: After the massacre at the Castle family reunion, Livia and Howard Saint go to the Saints and Sinners club. They give Micky Duka the newspapers where it says (in the lower left corner) that Frank’s 10-year-old son has been killed. But in the scene when Frank pulls his tombstone out of the ground, you can see the tombstone of his son William Castle where it says 1995-2003. That means he was only eight years old when he was killed, not 10 as it was stated in the newspapers.
- Continuity: There’s no blade left in the hilt when Castle uses his knife launcher (it’s in Heck’s neck). In the next shot, the blade reappears, and then disappears again in the following shot.
- Continuity: When Saint is tied to the car and the explosions are happening, his lower half catches fire. In the next shot, he is not on fire.
- Continuity: After Dave comes back from the hospital, the tissue in his nose switches sides after he comes into the apartment.
- Continuity: Castle shoots and damages the grill of the red truck as it attempts to run him down. When we see the truck again moments later, the grill is undamaged.
- Continuity: While being chased by a truckful of bad guys, the boat attached to Castle’s wife’s rig bounces up and smashes the red truck’s windshield. In the next shot, the windshield is intact.
- Continuity: After Saint’s wife puts in the left earring given to her by her husband she drops her robe. Then her right earring is seen in her ear.
- Continuity: The motorcycle lands pointing to the left when driver is shot off it, but when Castle picks it up to pursue the criminals, it’s facing the other way.
- Continuity: Both the barbecue and ketchup bottles move around on the grill several times during the shootout.
- Revealing mistakes: After Saint kills the henchmen that was assigned to look after his son, Saint drops the gun on the stomach of the henchman only to have him jump as the weight of the gun hit his gut.
- Revealing mistakes: After Castle shoots the gas tank, it blows up the henchmen setting him on fire and you can see the fireproof mask and gloves on the henchmen.
- Continuity: When Saint is being tied behind the car, his arms keep changing position from outstretched to being placed on his chest.
- Continuity: When one of the thugs comes down the stairs, he gets slashed by Castle’s knife, and blood gets splattered on the wall. The blood disappears in the next shot.
- Continuity: When the pictures of Quentin Glass are thrown at Howard Saint they can be seen as thrown above him and to his side, but in the next shot, they are all in a pile all facing upwards.
- Continuity: When Frank Castle is escaping from Harry Heck he drives his GTO off of the bridge. In the close-ups of Castle just before he hits the Chevy Lumina, you see him pull the steering wheel hard to the left, but in the scene the GTO goes right. He turns in the opposite direction the car actually goes.
- Crew or equipment visible: When the guitar player, Harry Heck, plays the song he wrote for Frank Castle, you can see a microphone on the guitar.
- Continuity: As Howard Saint is stabbing Quentin Glass, when the blade is shown twice after Glass is stabbed, the blade has no blood on it
- Crew or equipment visible: When the Jeep flips over we see the crew standing in the background.
- Crew or equipment visible: After Franks car jumps the bridge during the car chase, you can see the jump harness used to protect the stunt driver from receiving any back injuries.
- Continuity: In the initial FBI bust sequence, the drug dealer is holding a gun at his side and pacing around. When he finally acts, he reaches into his jacket to pull out the gun that he was already holding out in the shots before.
- Continuity: The line Castle attaches to the anti-personnel mine vanishes and reappears between shots.
- Continuity: When Glass blows up the dock, Castle is lying face down; however, moments before the explosion, we saw Castle begin to rise.
- Continuity: During the Shoot out at the beach house one of Saint’s men seeks refuge behind the BBQ which is riddled with bullet holes. In the next scene the bullet holes disappear.
- Continuity: During the chase scene, the red Ford pickup chasing Castle’s wife and son continually changes from standard to automatic.
- Continuity: When Frank is being chased by Heck, and Heck starts shooting at Frank’s car, all the bullet holes (caused by Heck) disappear as Frank is driving over the bridge. But, they reappear after the car flips.
- Continuity: In the gunfight between Castle and the man who is about to get blown up by the propane tanks, a salad bowl is clearly seen being toppled over and in the next shot it is normal again, and then shot.
- Continuity: As Castle is having dinner with Joan, Dave and Bumpo, right before his line, “Thanks for dinner,” his left hand is on the table and his right hand under the table on his leg. In the very next shot, both his hands are on the table.
- Revealing mistakes: When Quentin Glass is torturing Dave to find out where Castle is, he pulls out Dave’s eyebrow ring with a pair of pliers first, but when his hand jerks back, the piercing is not in the pliers.
- Revealing mistakes: At the beach shootout after Frank shoots the BBQ gastanks, you can see the victim’s fire protective headgear when he’s flying through the air.
